| Skills required | Active Luton are looking for a social prescription link worker to join the Family Hubs -Maternal Social Prescription Team.
This role will focus on supporting pregnant women, new parents and their newborn during the first 1000 days of life, providing 12 to 18 weeks of wellbeing and social prescribing interventions through our Family Hubs network. |

| Job role notes | The successful applicant will have experience of working in health, social or community care working in direct contact with service users. Good Communication skills, and be able to use Excel, Word and competently collect data and input it.
Travel between sites will be required and will include regular evening work and occasional weekends. |
